This simple questionnaire helps you check what processes and level of control you may need for each AI use.

Control includes:

  • who can make decisions
  • what approvals you need
  • how AI use is checked and managed. 

How it works

The questionnaire asks 7 questions about how you plan to use AI. Each question highlights examples where extra attention or control may be needed to manage risk.

Answer each question based on your current understanding. There are no scores or automatic outcomes.

If you answer yes to any question, that AI use or system may need stricter controls.

For example, an AI system that automatically handles customer complaints and refunds. A chatbot that handles restaurant bookings is generally lower risk and may need less oversight.

When to screen

Use the questionnaire before you develop a business case, invest in an AI solution or commit any resources.

This is a quick first step before doing a full risk assessment – not a replacement for one.
